THERAPY:
Initial Intake session, 60 Minutes: $180
On-going sessions, 45 Minutes: $165
Services are currently provided on a private pay, Lyra Health, or out-of-network insurance provider basis (see below). Telehealth sessions can be paid by a credit, Flex, or HSA card. In-person sessions have the additional payment options of check or cash.
Late cancellation: If a client fails to keep an appointment without notifying me 24 hours in advance, a $50 late cancellation fee will be charged.
Lyra Health Benefit: I am a covered provider with Lyra Health.
Out of Network Insurance: If you have out-of-network benefits, please contact your insurance company for eligibility and to see what percentage of the fee they cover. Using your insurance may limit confidentiality as the insurance company may request session information. They also may limit the number of sessions allowed. Please contact me if you have questions about this option.
Potential advantages of not using insurance:
- The number of sessions is not limited.
- Confidentiality. When you use your insurance, the insurance company’s case manager reviews your claims and has access to your clinical information.
- I can provide you with a statement that lists the session date and payment amount, which you might be able to submit to the following:
a. Your Health Savings Account (HSA) or Flex Spending Account for reimbursement. Please contact your benefits manager for verification and what documentation is required for reimbursement.
b. Your tax adviser for a possible tax deduction. Please consult with your tax advisor.
You have the right to receive a Good Faith Estimate explaining how much your medical care will cost
Under the No Surprises Act, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
- Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
- Make sure to save a copy or picture of your Good Faith Estimate.

COACHING:

Payment: Fees are payable at or before each scheduled session. Online coaching sessions are to be paid by a credit, Flex, or HSA card. In-person coaching sessions have the additional payment options of check or cash. I understand that there will be a $20.00 handling fee for all returned checks.
